HbA1c in Pregnancy — What It Means

HbA1c reflects your average blood sugar over roughly the past 8 to 12 weeks. It does this by measuring the share of hemoglobin with sugar attached. In early pregnancy, HbA1c can hint at your sugar control before you conceived and in the first trimester. This matters if you already have diabetes.

You may have this at your first antenatal visit if you have diabetes already or strong risk factors. It has a limited role later in pregnancy.
Why is it done in pregnancy?
In early pregnancy, HbA1c can hint at your sugar control before you conceived and in the first trimester. This matters if you already have diabetes.
About the reference range
There is no reliable HbA1c range set for each trimester in pregnancy. HbA1c is less reliable in the second and third trimesters, because faster red cell turnover shortens red cell life and can lower the value falsely. So the lab's printed range and your obstetrician's judgement apply. The OGTT stays the standard check for pregnancy diabetes.

- Pregnancy speeds up red blood cell turnover. Cells are replaced faster, so they spend less time with sugar attached. This shortens the memory window of HbA1c and tends to give lower values. So it is an unreliable sugar measure later in pregnancy.
- Can HbA1c be used to identify gestational diabetes?
- HbA1c is not advised for finding pregnancy diabetes. The 75 g OGTT is the set method. HbA1c's limits in pregnancy make it unsuitable for this.
- When is HbA1c useful during pregnancy?
- It is most useful in early pregnancy. This is true for women who already have diabetes. It reflects sugar control in the weeks around conception. This matters for judging early pregnancy risk.
- Does a high HbA1c early in pregnancy have any significance?
- A raised HbA1c in the first trimester can point to poorly controlled diabetes that was present before. It is linked with a higher chance of some pregnancy problems. Your obstetrician will discuss what it means and a care plan.
- How does HbA1c differ from a fasting glucose or OGTT?
- Fasting glucose and OGTT measure sugar at one moment or after a glucose drink. HbA1c reflects an average over weeks. In pregnancy, all three serve different purposes. They are not interchangeable.
