Ferritin is a protein that stores iron in your cells and tissues. Its blood level reflects the total iron reserves in your body. Your iron needs rise a lot in pregnancy. Low iron stores (low ferritin) can come before anaemia. Finding low ferritin early lets you top up iron before your hemoglobin falls.

When is it done?

Why is ferritin checked separately from hemoglobin?
Hemoglobin shows your current oxygen-carrying capacity. Ferritin shows your iron reserves. Ferritin can be low before hemoglobin drops. So ferritin can flag low iron at an earlier stage.
What is considered a low ferritin in pregnancy?
Many guidelines regard a ferritin below 30 ng/mL as low iron stores in pregnancy. Cut-offs vary between labs and guidelines. Your obstetrician will read your result in context.
Can ferritin be falsely normal even when iron stores are low?
Yes. Ferritin can rise with infection or inflammation. This can hide true low iron. Your obstetrician may add tests such as serum iron or transferrin saturation if there is concern.
Does routine iron supplementation in pregnancy make ferritin testing unnecessary?
Routine iron-folate supplements are advised for all pregnant women in India, whatever the ferritin. Ferritin testing helps flag women who may need higher-dose or vein-given iron. This is instead of standard supplements.
Will my ferritin recover after delivery?
Iron stores usually start to recover after birth. This is helped by taking supplements. It can take several months. Your obstetrician may advise you to keep taking supplements after delivery.
