Homocysteine — What It Means in Your Blood Test

New to reading lab reports? Start with our guide to reading a blood test report

This is a building block your body makes and clears with help from certain vitamins.

Your Homocysteine result reflects how well your body processes a common building block with the help of certain vitamins. It is one small clue among many. Doctors read it together with your wider picture, never on its own.

Also known as: tHcy, total homocysteine, plasma homocysteine, Hcy, homocystine

Kept in balance

This isn't a case where higher or lower is better. Your body keeps it in a narrow band, and both directions can matter.

What is Homocysteine?

Homocysteine is an amino acid formed as a by-product when cells break down the protein-derived compound methionine. Folate, vitamin B6, and B12 normally convert it onward, so your level reflects those vitamins and inherited enzymes.

Why is Homocysteine tested?

You may have this test because a higher level has been linked to strain on blood vessel walls. Some see it as a heart risk factor.

What is a normal Homocysteine range?

India
No India-specific national reference interval is published for this parameter; your lab's printed range is the most reliable comparison.
Global
Normal: 5–15 µmol/L; Mild hyperhomocysteinemia: 15–30 µmol/L; Moderate: 30–100 µmol/L; Severe: >100 µmol/L

Ranges vary by lab, method and equipment — the range printed on your own report is the one that applies to your result.

What does your Homocysteine value mean?

There is no widely-accepted set of value bands for this parameter. Compare your result with the reference range printed on your own report, and discuss it with your doctor.

What does a high Homocysteine result mean?

A higher homocysteine has been linked to stiffer arteries and stress on vessel walls in studies. It is worth discussing with your doctor.

What does a low Homocysteine result mean?

A lower homocysteine is usually seen as a good sign. It is not something that needs concern.

What can affect your Homocysteine result?

  • Low folate, vitamin B6, or vitamin B12 is the most common cause of a high level
  • Reduced kidney (renal) clearance lets homocysteine build up
  • Some inherited MTHFR gene forms can raise it
  • Smoking and lots of red meat are linked to higher levels
  • Levels tend to rise a little with age

In the Indian context

Indian vegetarian diets can be low in B12, which comes mainly from animal foods. This may raise homocysteine. Low B12 is common in Indian surveys.
